In California, a wage statement has ten essential pieces of information that must be provided:

  1. The employer’s legal name and address

  2. The name of the employee and the last four digits of his/her social security number or an employee identification number

  3. Net wages earned

  4. The inclusive dates of the pay period

  5. For employees paid on a piece-rate basis, the number of pieces rates unites earned, regardless of time worked

  6. All deductions

  7. Gross wages earned

  8. All applicable hourly rates in effect during the pay period and the corresponding number of hours worked at each hourly rate
